Mock Election Ballots
All ballots contain the same questions and possible responses. You don't need to print all the pages of the ballot if you have not prepared your students on all 5 ballot measures. There is one ballot measure per page (pp. 4-8), so you can easily print just the measures on which you want your students to vote.
The vote reporting form is the same for all ballots. You will likely ask your students not to vote on every question; they should only vote for the race for US Representative in their district and for the ballot measures for which you have prepared them. This means you will have blanks on the vote reporting form. This is fine!
Please do not fax this form since there will be no one to collect your votes at the fax number. You need to submit your votes by email, as instructed. Thanks!

Using the Oregon Student Mock Election Ballot with an OMR answer form
There are two possible ways to process the Mock Ballot with an OMR machine. You can use a ballot form or a survey form. A mock ballot has been created for each. Please use the appropriate forms with the appropriate ballot. OMR forms can be purchased from www.pearsonNCS.com, www.scantron.com, or other sites.
Ballot Form: You will need to find a ballot form that allows for at least 78 candidates, or a survey form that allows for at least 6 response choices and at least 20 questions.
